Introduction: Adolescent obesity is a global issue with widespread consequences. Self-efficacy, as a core construct in healthy lifestyle behavior change, plays an important role in obesity prevention. This systematic review investigated the impact of community-based obesity prevention interventions on adolescent self-efficacy. Materials and Methods: We conducted a systematic search in PubMed, Scopus, Web of Science, ProQuest, PsycINFO, and CINAHL for trial studies (randomized or quasi-experimental) published between January 2015 and January 2026. Eligible studies were community-based trials that measured self-efficacy as an outcome among adolescents aged 10–19 years. Data extraction and quality assessment were conducted independently by two reviewers. Results: From 8,522 studies, 8 studies with 4,571 participants from five countries (USA, Australia, Brazil, Poland, Hong Kong, and China) were included. Results were heterogeneous. Three studies (from Hong Kong, the United States, and China) reported significant improvements in self-efficacy; one study (Australia) showed short-term improvement; two studies (Brazil and Australia) observed no significant change; and one study (Poland) reported a decrease in self-efficacy. Studies conducted exclusively on girls showed no improvement, and the Polish study even showed decreased self-efficacy in girls. Conclusion: Community-based obesity prevention interventions may exert varying and sometimes unstable effects on the self-efficacy of adolescent boys and girls. Addressing these differences necessitates gender-sensitive, community-based designs with motivational and supportive components to improve adolescent self-efficacy sustainably. |
