Association between Plant-Based Diet Index Adherence and the Risk of Cardiovascular Disease among Adults with Multimorbidity: A Cohort from Tehran Lipid and Glucose Study

Introduction: In recent years, many cohort studies have investigated the association between plant-based diets and cardiovascular disease (CVD) risk. However, very few studies have examined the association between adherence to plant-based diet indices and CVD risk in individuals with multiple comorbidities. Therefore, the present study was designed to investigate this association. Materials and Methods: In this cohort study, 1688 adults were selected from the Tehran Lipid and Sugar Study. Participants' dietary intake and adherence to the plant-based diet index (PDI) were assessed using a food frequency questionnaire. The association between PDIs and CVD incidence was examined using Cox proportional hazards regression. Results: Participants' mean age was 43.4 years, and 53.7% were women. During the 10-year follow-up, 166 cases of cardiovascular disease were identified. After adjustment for all potential confounders, the hazard ratio (95% confidence interval) for CVD per standard deviation (SD) increase in the overall PDI was 0.94 (0.80-1.11), the healthful PDI (hPDI) was 0.85 (0.71-1.02), and the unhealthful PDI (uPDI) was 1.24 (1.04-1.46). Based on the restricted cubic spline, the observed association between uPDI and CVD was nonlinear. Conclusion: Higher adherence to uPDI appears to be associated with an increased risk of CVD in individuals with multiple comorbidities. Further well-designed cohort studies across different populations are recommended to inform this decision. |
