The Role of Remnant Cholesterol in Predicting the Incidence of Type 2 Diabetes: A Narrative Review of the Existing Evidence

Type 2 diabetes is a prevalent metabolic disorder and a major contributor to the global burden of disease. Identifying effective biomarkers for predicting the onset of this condition can aid in targeted prevention and treatment strategies. Remnant cholesterol (RC), primarily found in triglyceride-rich lipoproteins (TRLs) such as VLDL and IDL, plays a significant role in athero­sclerosis and cardiovascular disease and has recently attracted attention for its association with metabolic disorders and type 2 diabetes. In this narrative review, all prospective cohort studies examining the association between RC levels and the incidence of type 2 diabetes were identified through searches in MEDLINE (PubMed), Web of Science, and Scopus as of July 15, 2025. Their findings indicated a significant association between elevated RC levels and an increased risk of developing type 2 diabetes, particularly in Asian populations. However, the strength of this association tends to diminish after adjusting for confounding factors. Furthermore, while some studies have identified RC as the most predictive component of the lipid profile for type 2 diabetes, others have pointed to triglycerides (TG) as a stronger predictor. The findings of this review underscore the potential utility of RC as a biomarker for predicting type 2 diabetes.They may inform the development of novel strategies for early prevention and intervention. Nonetheless, current studies are limited by variable sample sizes and a predominant focus on the Asian population, underscoring the need for further research in larger and more diverse populations to validate and generalize the findings.
