Assessment of Osteoporosis Risk Status and Associated Factors among Iranian Adults

Introduction: Osteoporosis is recognized as a major public health problem with severe complications, particularly in developing countries like Iran. This study assessed the risk factors associated with osteoporosis among Iranian adults. Materials and Methods: This cross-sectional study was conducted in autumn 2023 among 1009 adults aged 30 and over with internet access, via an in-person survey. We collected data online using the International Osteoporosis Foundation risk assessment checklist. Participants who answered "yes" to at least one of the 8 risk assessment questions were classified into the "high-risk" group. Data analysis was performed using SPSS software. Results: Among the 1009 participants (81.3% female, median age 48 years), 58.3% were in the high-risk group for osteoporosis. Multivariate analysis revealed that the odds of being in the high-risk group were 2.27 times higher in men than in women (95% CI: 1.49-3.45), 2.94 times higher in individuals aged ≥50 years compared to younger individuals (95% CI: 2.14-4.03), and 2.13 times higher in unemployed individuals than in employed ones (95% CI: 1.44-3.14). University education was protective, reducing the odds of high risk by 33% (OR=0.67, 95% CI: 0.45-0.99). Conclusion: The findings confirm a high prevalence of osteoporosis risk factors in the studied population. Older age, male gender, unemployment, and lower education level were identified as independent predictors of high risk. These results highlight the need to implement targeted educational programs and active screening, especially for high-risk groups.
