Role of Zinc in Prediabetes: A Review

Prediabetes is defined as having blood glucose levels above normal but below the diabetes thresholds. The increasing prevalence of Prediabetes is a global public health issue. Prediabetes increases the risk of developing some non-communicable diseases, including type 2 diabetes, cardiovascular disease, high blood pressure, and fatty liver disease. Lifestyle intervention is the first-line treatment against Prediabetes progression. The importance of micronutrients in the diet in Prediabetes's pathogenesis and regression/progression has yet to be discovered This review aims to investigate the metabolism of zinc in the body, its role in glucose and insulin homeostasis, and the pathogenesis, regression, or progression of Prediabetes. The results of the studies indicate the critical role of zinc in the metabolism and homeostasis of glucose and insulin. Observational studies show that both deficiency and overload of zinc are effective in the pathogenesis and progression of Prediabetes. It is known that individuals with hyperglycemia have dysfunction of zinc transporters in kidney tubules and intestine, as a result, increased urinary excretion of zinc and decreased intestinal absorption of zinc, and therefore, zinc deficiency and disturbance in glucose and insulin homeostasis. Maintaining the optimal zinc level is necessary to prevent the progression of Prediabetes. Findings of clinical trials indicate that zinc supplementation in Prediabetes patients is effective in reducing fasting blood sugar, two-hour blood sugar, glycosylated hemoglobin, insulin resistance and increasing the function of beta cells, insulin sensitivity, as well as improving the lipid profile and preventing the progression of Prediabetes to diabetes.
