fa ارتباط مثبت فنوتیپ دور کمر هیپرتری‌گلیسریدمیک با اختلال متابولیک در افراد با سابقه‌ی فامیلی دیابت Relationship Between Hypertriglyceridemic Waist Phenotype with Metabolic Disorders in Individuals with Family History of Diabetes پژوهشی Original مقدمه: از بین روش‌های غربالگری، امروزه بیشتر از شاخص‌های تن‌سنجی به همراه سایر شاخص‌های بیوشیمیایی استفاده می‌شود. مطالعه‌ی حاضر با هدف بررسی ارتباط فنوتیپ EWET) Enlarged Waist Elevated Triglyceridemic) با اختلال‌های متابولیکی در افراد با سابقه فامیلی دیابت صورت گرفت. مواد و روش‌ها: این مطالعه مقطعی در 1323 فرد (332 مرد و 991 زن) 55ـ35 ساله انجام شد. وزن، قد و دور کمر طبق روش‌های استاندارد اندازه‌گیری شد. نمونه‌ی خون ناشتا گرفته و آزمایش‌های قند و چربی‌های خون با استفاده از کیت‌های تجاری انجام شد. هم‌چنین آزمون تحمل گلوکز خوراکی انجام شد. اختلال تحمل گلوکز به صورت 2002hpG< ≤ 140 میلی‌گرم در دسی‌لیتر و دیابت به صورت 126FBS≥ یا 200≥ 2hpG میلی‌گرم در دسی‌لیتر در دو آزمایش متوالی تعریف شد. فنوتیپ دور کمر هیپرتری گلیسریدمیک (EWET) به صورت داشتن همزمان دور کمر بزرگ‌تر از 102 در مردان و 88 سانتی‌متر در زنان و تری‌گلیسرید بزرگ‌تر از mg/dL 150 میلی‌گرم در دسی‌لیتر در هر دو جنس تعریف شد. یافته‌ها : شیوع فنوتیپ دور کمر هیپرتری گلیسریدمیک در مردان 6/9% و در زنان 6/23% بود. مردان و زنان مبتلا به فنوتیپ EWET، دارای میانگین شاخص‌های تن‌سنجی بالاتری در مقایسه با سایر گروه‌ها بودند. در مردان پس از تعدیل اثر سن و فعالیت فیزیکی، افراد مبتلا به EWET مقادیر بالاتر، کلسترول (001/0P<)، فشارخون سیستولی (001/0) و دیاستولی (001/0) داشتند. تفاوت‌ها پس از تعدیل اثر BMI معنی‌دار باقی ماندند اما در مورد HDL این ارتباط مشاهده نگردید. زنان مبتلا به EWET قبل و بعد از تعدیل BMI میانگین بالاتری از قند، چربی‌های خون و فشار خون داشتند (001/0P< برای همه). افراد مبتلا به EWET دارای شیوع بالاتر دیابت (در هر دو جنس) و IGT (در زنان) بودند (05/0P< برای همه). نتیجه‌گیری: یافته‌ها حاکی از ارتباط مثبت فنوتیپ EWET با بیماری دیابت است. استفاده از این فنوتیپ برای شناسایی زودرس افراد در معرض خطر دیابت و اختلال تحمل گلوکز خوراکی پیشنهاد می‌شود. Abstract:Introduction: Recent studies have used antropometric measures along with biochemical indicators as screening tools for chronic diseases. Few studies have assessed the relation of Enlarged Waist Elevated Triglycerdemic (EWET) phenotype with diabetes, particularly among individuals with family history of diabetes. This study aimed to evaluate the association of the EWET phenotype with diabetes among individuals with family history of diabetes. Material and Methods: Antropometric and biochemical measurments were assessed in a pouplation-sbased cross-sectional study of 332 male and 991 female Isfahani adults, aged 35-55 years. The EWET phenotype was defined as serum trigcylglycerol concentrations ≥ 150 mg/dl and concurrent waist circumference ≥ 88 cm in women and ≥ 102 cm in men. Results: The prevalence of the EWET phenotype was 9.6% among men and 23.6% among women. Individuals with the phenotype had significantly higher antropometric measurements as compared to other groups. After control for age and physical activity, men with the EWET phenotype were significantly more likely to have high serum cholestrol (P<0.001), systolic BP (P<0.01) and diastolic BP (P<0.001). Even after aditional control for BMI the significant associations remained except for low HDL Cholestrol women with the EWET phenotype, who had significantly adverse metabolic risks as compared to other phenotypes, both before or after control for BMI (P<0.001). Individuals with the EWET phenotype were more likely to have diabetes (both genders) and IGT (women only). Conclusion: Our findings showed a significant association between the EWET phenotype and diabetes.
