fa راهنمای بالینی و توصیه های تشخیص و درمان پوکی استخوان و کم‌ماهیچگی: خلاصه اجرایی Iranian Guidance and Recommendation for the Diagnosis and Treatment of Osteoporosis and Sarcopenia: Position Statement Executive Summary غدد درون‌ریز Endocrinology مروری Review <div style="text-align: justify;">مقدمه: پوکی استخوان به‌عنوان شایع‌ترین بیماری متابولیکی استخوان، یک بیماری‌ مزمن و یک تهدید اقتصادی- اجتماعی است. هدف از این مطالعه تدوین راهنمای بالینی پوکی استخوان به منظور یکپارچه‌سازی ارایه خدمات می‌باشد. مواد و روش‌ها: این مطالعه در دو مرحله؛ شامل مرور جامع و روش گروه خبرگان، طراحی و انجام شد. ابتدا جستجوی جامع منابع در بانک‌های اطلاعاتی معتبر انجام شد. سپس در بخش گروه خبرگان، متخصصین با امتیازدهی، قضاوت نهایی خود را در خصوص مزیت‌ بالینی و قابلیت بومی‌سازی هر یک از توصیه‌ها به‌صورت «کم، متوسط و زیاد» مشخص نمودند. در نهایت از بین توصیه‌های متفاوت برای یک سؤال بالینی، توصیه‌ای که بیش از 70 درصد از اعضای گروه خبرگان به آن رای مثبت داده بودند، به‌عنوان توصیه نهایی انتخاب شد. یافته‌ها: با بررسی راهنماهای بالینی معتبر دنیا، 5 راهنما با توجه به معیارهای غربالگری مطالعه انتخاب و وارد مطالعه شدند. پس از برگزاری چندین جلسه‌ی گروه خبرگان و بررسی توصیه‌ها در نسخه‌ی اولیه، 28 سؤال و 181 توصیه استخراج شد. پس از کسب نظرات صاحب‌نظران، توصیه‌های نهایی در یک جلسه با حضور متخصصین بالینی مورد بررسی قرار گرفت و بر اساس نظرات ارایه شده، نسخه‌ نهایی و به‌روزرسانی شده راهنما، در قالب 34 سؤال و 111 توصیه در حوزه سلامت استخوان (4 سوال، 12 توصیه)، تشخیص و ارزشیابی (6 سوال، 19 توصیه)، درمان و توانبخشی (17 سوال، 67 توصیه) و کم‌ماهیچگی (سارکوپنی) (7 سوال 13 توصیه) ارایه شد. نتیجه‌گیری: با توجه به تاکید بر ضرورت دسترسی به دستورالعمل‌های معتبر برای ارائه‌ی خدمات بهینه و سودمند به بیماران این مرز و بوم و رفع چالش‌های شکاف مراقبت، اقدام به تدوین راهنمای بالینی پوکی استخوان شده است.</div> <div style="text-align: justify;">Introduction: Osteoporosis is the most common metabolic bone disease. It is a chronic disease and a socio-economic threat. The aim of this study was to develop a clinical guideline for osteoporosis to integrate service delivery. Materials and Methods: This study was designed and conducted in two phases: comprehensive systematic review and expert panel method. First, a comprehensive search of sources was conducted in reliable databases. Then, in the expert panel phase, the clinical questions and recommendations were presented to members of the panel of experts. Finally, among various recommendations for a clinical question, the recommendation, which was approved by more than 70% of the panel of experts, was chosen as the final recommendation. Results: By reviewing the valid guidelines of the world, five guidelines were selected according to the screening criteria of the study and entered into the study. After holding several sessions with the panel of experts and reviewing the recommendations in the initial version, 28 questions and 181 recommendations were extracted. After obtaining the opinions of professors from different disciplines, the final recommendations were reviewed in a meeting with clinical experts, and based on the submitted opinions, the final version of the guideline update, in the form of 34 questions and 111 recommendations in the field of bone health (four questions, 12 recommendations), diagnosis and Evaluation (six questions, 19 recommendations), treatment and rehabilitation (17 questions, 67 recommendations), and Sarcopenia (seven questions, 13 recommendations), were presented.
