fa دیابت بارداری در زنان مبتلا به بیماری پریودونتیت: یک مطالعه کوهورت Gestational Diabetes Mellitus in Women with Periodontitis Disease: A Cohort Study زنان Gynecology پژوهشی Original مقدمه: دیابت حاملگی (GDM) از شایع‌ترین اختلالات متابولیک است که برای اولین بار در دوران بارداری تشخیص داده می‌شود. عفونت پریودنتال نیز التهابی موضعی و مزمن است که می‌تواند بعنوان یک شروع‌کننده پاسخ التهاب سیستمیک در نظر گرفته شود و برخی معتقدند که با بروزGDM مرتبط است. با توجه به شواهد ناهمگون در این باره، هدف از مطالعه ی آینده‌نگر حاضر تعیین ارتباط بین بیماری پریودنتیت و GDM می‌باشد. مواد و روش‌ها: این مطالعه به صورت کوهورت در درمانگاه پره‌ناتال بیمارستان روحانی بابل، در شهریور 1398 لغایت شهریور 1399 انجام شد. وضعیت پریودنشیوم زنان باردار از نظر شاخص‌های خونریزی در اثر پروبینگ (BOP)، عمق پاکت پریودونتال (PPD)، میزان از دست رفتن چسبندگی(CAL)  و شاخص لثه‌ای Gingival Index(Loe & Silness)، در هفته 12 تا 24 بارداری توسط یک دندان‌پزشک در همان درمانگاه بررسی و موارد بیماری تشخیص داده شد. سپس در هفته 24 تا 28 بارداری، بر اساس آزمایش تست تحمل گلوکز 75 گرمی، ابتلا به GDM مورد ارزیابی قرارگرفت. یافته‌ها: بروز GDM در زنان مبتلا به پریودنتیت بیشتر بود (0/001P<). میزان خطر نسبی خام ابتلا به دیابت بارداری در زنان مبتلا به پریودنتیت با (0/001P< 95% CI) (21/77-6/54) 11/94 و خطر نسبی تعدیل شده با متغیرهای شاخص توده بدن، دور کمر، تعداد حاملگی و تعداد زایمان با (0/001P< 95% CI) (20/58-6/12) 11/23 بوده است. میانگین GI، PPD، BOP، CAL بطور معنی‌داری در زنان باردار مبتلا به GDM بیشتر از گروه سالم بود (0/05>P). نتیجه‌گیری: جهت پیشگیری از بروز دیابت بارداری سیاست تشخیص زودرس و درمان پریودنتیت باید در الویت مراقبت پره‌ناتال و حتی قبل از باردار شدن قرار گیرد. Introduction: Gestational diabetes mellitus (GDM) is one of the most common metabolic disorders firstly diagnosed in pregnancy. Periodontal infection is also a localized and chronic inflammation that can trigger the systemic inflammatory response, and some believe it is associated with GDM. There is insufficient evidence to support a link between periodontal disease and GDM. This prospective study aimed to determine the relationship between periodontal disease and GDM. Materials and Methods: This cohort study was performed in the Prenatal Clinic of Rouhani Hospital in Babol from September 2018 to September 2019. The periodontal status of pregnant women in terms of bleeding on probing (BOP), periodontal pocket depth (PPD), clinical attachment loss (CAL), and gingival index (Loe & Silness) at 12 to 24 weeks of gestation by one dentist was examined in the same clinic to diagnose periodontitis cases. Then, at 24 to 28 weeks of gestation, GDM was evaluated based on the 75 g glucose tolerance test. Results: The incidence of GDM was higher in women with periodontitis (P<0.001). The raw relative risk of GDM was 11.94 (CI 95%: 6.21-54.77, P<0.001). The relative risk, adjusted for body mass index, waist circumference, number of pregnancies, and number of deliveries, was 11.23 (CI 95%: 6.20-12.58, P<0.001). The mean GI, CAL, BOP, and PPD were higher in pregnant women with GDM than in the healthy group (P<0.05).
