fa کارایی مداخلات درمانی در طبیعی شدن رشد کودکان مبتلا به کم‌کاری مادرزادی تیرویید تشخیص داده شده به روش غربالگری نوزادی غدد درون‌ریز Endocrinology پژوهشی Original مقدمه: هدف پژوهش حاضر، بررسی کارایی مداخلات درمانی انجام شده روی کودکان مبتلا به کم‌کاری مادرزادی تیرویید از راه بررسی روند رشد، در قالب تعیین صدک‌های قد، وزن، دور سر و مقایسه‌ی آن‌ها با کودکان طبیعی (براساس گزارش WHO) بود. مواد و روش‌ها: در این پژوهش هم‌گروهی آیندهنگر، 760 کودک مبتلا به کم‌کاری مادرزادی تیرویید تشخیص داده شده، به روش برنامه‌ی غربالگری نوزادی در اصفهان وارد پژوهش شدند. صدک سوم (متناظر با 2- =z-score) متغیرهای قد، وزن و دور سر این کودکان محاسبه، و با مقادیر متناظر در کودکان طبیعی مقایسه گردید. آزمون تک نمونه‌ای نسبت و آزمون مجذور خی با استفاده از نرم‌افزار SPSS برای تحلیل داده‌ها استفاده گردید. یافته‌ها: صدک‌های قد، وزن و دور سر کودکان مبتلا، با کودکان طبیعی متفاوت بود (01/0P<). درصد فراوانی نسبی z-score، 2- و کمتر (رشد مختل) برای متغیر قد به ترتیب به 3% و 9% برای کودکان دختر و پسر در انتهای 5 سال پی‌گیری رسید. فراوانی نسبی z-score 2- و کمتر برای متغیر دور سر برای هر دو جنس در انتهای سه سالگی به کمتر از 3% رسید. با این‌که درصد بیماران دارای اختلال رشد در متغیر وزن در طول دوره‌ی درمان دارای روند نزولی بود، اما تفاوت آن با 3% به لحاظ آماری معنی‌دار نبود. نتیجه‌گیری: پژوهش حاضر نشان داد مداخلات درمانی منجر به طبیعی شدن روند رشد متغیرهای قد، وزن و دور سر در کودکان مبتلا می‌گردد؛ هر چند در مورد وزن دیرتر و در مورد دورسر زودتر چنین اتفاقی روی می‌دهد. Introduction: The aim of this research was to determine the efficacy of therapeutic interventions applied to children with congenital hypothyroidism by evaluating of the growth status of the children and comparing the percentiles of their height, weight and head circumference with normal infants reported by WHO. Materials and Methods: In this prospective cohort study, 760 congenitally hypothyroid neonates diagnosed and followed up during the CH screening program in Isfahan were enrolled. The 3rd (i.e. Z score of -2), percentile for height, weight and head circumference of both sexes was determined and compared with the corresponding WHO values. Chi-square and one sample proportion statistical test using SPSS statistical software were used for analyzing data. Results: The percentiles of weight, height and head circumferences of the patients studied were statistcally different from WHO values (P<0.01). The relative frequency ≤-2 Z scores for height in both male and female CH patients reached to between 3% and 9% at 5 years of age respectively. The relative frequency of ≤ -2 Z scores for head circumference reached to <3% at 3 years of age in both sexes. While the weight showed a decreasing trend during the treatment period, it was not significant below 3%.
