Mid-term Prospective Analysis of Renal Filtration Function after Bariatric Surgery in Adults with Type 2 Diabetes

Introduction: Obesity is associated with a range of medical complications, including impaired renal function. Type 2 diabetes mellitus (T2DM) exerts both direct and multifactorial impact on renal function. This study aims to evaluate the impact of metabolic bariatric surgery (MBS) on renal function in adults with T2DM. Materials and Methods: In this prospective cohort study, patients who underwent MBS were followed for three years, and postoperative trajectories of creatinine-based estimated glomerular filtration rate (eGFR) were assessed. Generalized estimating equations (GEE) were used to evaluate longitudinal changes in eGFR over time. Additionally, uni-variable and multivariable linear regression analyses were performed to identify factors associated with eGFR changes during follow-up. Results: A total of 514 participants (85.4% women; mean BMI: 44.9 kg/m2) were included. All participants had a baseline eGFR≥60 mL/min/1.73m2. eGFR showed significant improvement over the three-year follow-up period. In multivariable analysis, older age, female sex, higher baseline eGFR, and undergoing sleeve gastrectomy (SG) were identified as negative predictors of postoperative eGFR improvement. Conclusion: Renal function improved following MBS, in adults with T2DM. Furthermore, older age, female sex, higher baseline eGFR, and undergoing SG were associated with less eGFR improvements.
